TRAINEES (Residents, PhD students, Fellows) and Allied Health Personnel
Student status and allied health personnel must be certified by a Head of Department, and a copy of an ID card or passport must be submitted with the registration.

NAME CHANGE
Should you be prevented from attending, you may be substituted by a colleague. An administrative fee of USD 25 will be charged.

GROUP REGISTRATIONS

Group registrations will be offered to groups of 10 persons or more. In order to make a group registration, please send an E-mail with an excel sheet with all the participants' names together with your postal address and contact details. Please note that we need the complete individual addresses (hospital, department, street name and number, postal code, city, e-mail address) of each participant. Before the start of the Congress, appointments for pre-pickup of the Congress material can be made for pre-registered groups. Upon receipt of the correct registration fee, each participant will receive a confirmation of registration.

CANCELLATION AND REFUND POLICY

Registration Fees will be refunded upon receipt of a written notice of cancellation as follows:

Before and On August 15, 2008                             50% refund
After August 15, 2008                                              No refund

Note: Refunds will NOT be made for NO SHOW

CERTIFICATE OF ATTENDANCE
A certificate of attendance will be issued to all Full Delegate attending the entire Conference
